The Models of Care Portal is a web platform developed as part of the programme to support workforce sustainability. The unique online resource is available across the NHS to aid learning, share tools and the achievements of providers across the whole of health and care.

Features include:

• General Practice Workforce Analysis Tool hosted and accessible to registered users.
• Collaboration Space secure, safe space for private discussion between groups, document sharing and editing.
• Interactive Map overlaid with STPs/CCGs/General Practices/Projects.
• Resources organised against the 10 High Impact Actions identified in the General Practice Forward View.
• Case Studies linked to the 10 High Impact Actions.
• One 2 One messaging for private conversations with other users to aid learning.
